Yancey Health Department COVID-19 Vaccine Application

Here are the steps you will take in order to let the health department know you’re interested in getting the vaccination.

  1. Complete the Yancey Co Health Dept form in this LINK. The form will also be posted on the Yancey County Website.
  2. When complete and submitted, your form will be automatically uploaded to the TRHD.
  3. Wait for an email from NC Dept of Health and Human Services (NC DHHS) inviting you to create an account in the COVID Vaccine Management System. This might take a while.
  4. Once you create your account, you will receive an auto generated reply from NCDHHS informing you which priority group you are in.

Other things that are good to know: 

  • The priority group you will be placed in is totally dependent on the information you provide to NC DHHS.
  • The Yancey County Health Dept has no authority to determine your priority group.
  • The health department form is not required to receive a vaccine.
  • There is no cost for the vaccine.
  • If you have insurance, you may be asked for your insurance information to be billed for an administrative fee to cover a small portion of our nursing costs but there will be no charge to you.
