Here are the steps you will take in order to let the health department know you’re interested in getting the vaccination.
- Complete the Yancey Co Health Dept form in this LINK. The form will also be posted on the Yancey County Website.
- When complete and submitted, your form will be automatically uploaded to the TRHD.
- Wait for an email from NC Dept of Health and Human Services (NC DHHS) inviting you to create an account in the COVID Vaccine Management System. This might take a while.
- Once you create your account, you will receive an auto generated reply from NCDHHS informing you which priority group you are in.
Other things that are good to know:
- The priority group you will be placed in is totally dependent on the information you provide to NC DHHS.
- The Yancey County Health Dept has no authority to determine your priority group.
- The health department form is not required to receive a vaccine.
- There is no cost for the vaccine.
- If you have insurance, you may be asked for your insurance information to be billed for an administrative fee to cover a small portion of our nursing costs but there will be no charge to you.
